Ask clients to agree to a consent form

Write the forms clients agree to before a service, such as a patch test or a health questionnaire, attach them to services, and record a form signed at the desk.

  1. 01

    Write the form

Open Bookings › Consent forms and press “New form”; it is there if your role may change bookings settings. Give it a “Name” and the full “Text”. The text is plain, shown in full on your booking page and stored with every agreement.

  2. 02

    Attach it to services

Under “Asked for by”, tick the services that need it; the service's own “Consent” tab does the same. Press “Create”, or “Save” when editing.

  3. 03

    Know what the client does

When a client books one of those services online, they read the form and agree to it with their name before they can finish. Each agreement keeps a copy of the text, the name and the version the client agreed to.

  4. 04

    Change the wording

Edit the form and raise its “Version” when the wording changes in a way that matters. Earlier agreements keep the version the client saw.

  5. 05

Record a form signed at the desk

On the client's record, under “Consent”, press “Record consent”, pick the “Form”, type the “Name as given” and press “Save”. When the client has children, pets or vehicles on file, “For whom” records which one the form was signed for.

  6. 06

    Retire a form

A form clients have agreed to cannot be deleted. Switch off “In use” instead; it shows as “Retired” and is no longer asked for.

  7. 07

    Forms for a child

When a visit or a place in a class is for a child, the form is signed by the adult on the child’s behalf, as parent or guardian, and Setvos refuses the booking until the forms its services ask for are signed. On your booking page and in the Setvos app, the parent agrees on the child’s behalf; at the desk, record the form for that child under “Consent” first.

Common questions

Can the client sign an agreement with Дія.Підпис?

Yes, when signing with Diia.Signature is available on your account. On the client's record, press the signature icon (“Sign with Diia.Signature”) beside an agreement, then “Ask for a signature”; the client opens the link or scans the code with their phone and confirms in the Diia app. The signature is Diia's; Setvos keeps the document as it was sent and the signature Diia returns.

Can I write consent forms on the phone?

Yes. In Setvos Business, More → Bookings → Consent forms lists each form with its title, its version and the services that ask for it; retired forms are listed apart. The editor has a long text, the services and the note “Bump the version when the wording changes — a signature keeps the version it signed.” Removing a form someone has signed retires it instead of deleting it. On an iPad the list sits beside the editor.

How do clients sign a consent form in the app?

Before booking, each form opens in full; once it is read, the client types their full name and signs. The signature keeps the text of the form as it read.
